THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.
Effective date: October 15, 2026
Our Responsibilities
Entelos Behavioral Health LLC is required by law to protect the privacy of your health information, to give you this notice of our legal duties and privacy practices, to follow the terms of the notice currently in effect, and to notify you if a breach of your unsecured health information occurs. Your records are protected by the federal HIPAA Privacy Rule and California law, including the California Confidentiality of Medical Information Act. If you receive treatment for a substance use disorder, those records are also protected by federal confidentiality rules (42 CFR Part 2). Where these laws differ, we follow the one that gives you more protection.
How We May Use and Share Your Information
We may use and share your health information for the following purposes. For substance use disorder records, we first need your written consent, which may be a single consent covering treatment, payment and health care operations. You may revoke a consent in writing at any time, except where we have already relied on it.
- Treatment: sharing information with the people involved in your care.
- Payment: billing and getting payment from your health plan or other payers.
- Health care operations: running our program, improving quality and training staff.
In limited situations allowed or required by law, including:
- Medical emergencies, to the extent needed to treat you
- Reporting suspected child or elder abuse or neglect as required by state law
- Preventing a serious and imminent threat to someone’s health or safety
- Audits and program evaluations, under strict confidentiality conditions
- Research, under the conditions federal law requires
- Court orders and other legal processes, subject to the additional limits that apply to substance use disorder records
- Reporting to public health authorities as permitted by law
Substance use disorder records and legal proceedings. If you receive substance use disorder treatment, those records, or testimony about them, may not be used or disclosed in a civil, criminal, administrative or legislative proceeding against you unless you consent in writing or a court orders it after notice and an opportunity to be heard, as federal law requires.
Other uses. We will not sell your information or use it for marketing, and we will not share psychotherapy notes, without your written authorization.
